The New York State Council of Health-system Pharmacists (NYSCHP) recognizes members and non-members for achievements and accomplishments in the pharmacy profession.

Student Corporate Sponsor Essay Topic for 2017

Please submit your essay regarding the following topic:
How can the Pharmaceutical Industry support Pharmacists in assisting with "transition of care" within Health Systems?

 

NYSCHP Awards:

Pharmacy Achievement (FKA Board of Directors) Award:  The recipient and one (1) guest will receive a complimentary registration to the NYSCHP Annual Assembly, meals and accommodations for one night.  The award will consist of lifetime membership as a Honorary Member in the NYSCHP and a bronzed plaque.


Procedures and Guidelines:  The Pharmacy Achievement Award is intended to honor the accomplishments and contributions of a member of the NYSCHP by the Board of Directors.  The President will request input from each member of the Board of Directors with regard to possible candidates.  Each candidate must provide a recent version of his or her CV for review.  The Executive Committee will confer to determine the candidates for this award. Prior to presentation, the Board of Directors will vote on the candidate(s) to determine an award recipient.

Criteria:  The NYSCHP Pharmacy Achievement award shall be given to an individual of high moral character, good citizenship and high professional ideals, who has made significant contributions to health-system pharmacy; or an outstanding single achievement; or a combination of accomplishments benefiting health-system pharmacy, and through it, humanity and the public health.  They may include accomplishments, achievement or outstanding performance in:
        -health-system pharmacy practice;
        -health-system pharmacy education;
        -health-system pharmacy administration;
-pharmaceutical research or development related to health-system    pharmacy;
-scientific or professional pharmaceutical writing, e.g., noteworthy published articles on pharmaceutical subjects;
        -pharmaceutical journalism related to health-system pharmacy;
-public and/or inter-professional relations activities benefiting health-system pharmacy;
-pharmaceutical law or legislation, professional regulations, standards of professional conduct of pharmacy law enforcement.
-contributions to the function and programs of the NYSCHP

Presentation:  The award will be presented at the Annual Assembly installation banquet. The recipient may offer brief remarks upon acceptance of the award.

This award does not need to be given every year.


Corporate Scholarship Essay Award: The recipient will receive a complimentary registration to the NYSCHP Annual Assembly and accommodations for one night.  The award will consist of a $1500 scholarship awarded by the corporate sponsors and a framed certificate

Criteria:    Recipient must be in final two (2) years of Pharmacy School; Recipient must be attending a NYS Pharmacy School; Student must compile a 1000 word essay based on a topic chosen by the NYSCHP Resource Development Committee (RDC).

Procedures and Guidelines: Essays should be submitted to the school faculty liaisons who will then forward to the Council office.  The RDC Chair will compile all of the entries and submit them to the entire committee along with an accompanying assessment sheet for evaluation.

Presentation:  The award will be presented at the Annual Assembly installation banquet.


Corporate Achievement Award:  The recipient will receive a complimentary registration to the NYSCHP Annual Assembly, meals and accommodations for one night.  The award will consist of a plaque.

Procedures and Guidelines:  The Corporate Achievement Award is given by the NYSCHP to a company or corporate individual in recognition of accomplishments for, involvement in and betterment of NYSCHP.  Nominations are made by members of JCI and RDC and submitted to the NYSCHP Awards Committee.

Awards Committee:  The Chair will compile the entries and send copies to each Award Committee member with an accompanying assessment sheet for evaluation.  The Awards Committee will forward completed assessment sheets to the Chair for review and compilation. A summary will be presented to the NYSCHP BOD for a vote at the next BOD meeting after the application deadline.

Criteria:  Speaker sponsorship, involvement within committee; Other activities that better the organization; Minimum of 3 years with organization; Suggested 3 years with committee work.  Not intended to be a retroactive award:  company/individual   recognized with 1 year of achievement and still active. Check list of accomplishments.

Presentation:  The award will be presented at the Annual Assembly installation banquet.  

This award does not need to be given every year.

New Practitioner of the Year Award:  The recipient will receive a complimentary registration to the Annual Assembly and accommodations for (1) one night.  The award will consist of a wood plaque.
 
Procedure and Guidelines:  The new practitioner of the year will be presented to a deserving member of the pharmacy community of New York State.    The Presidents of the NYSCHP local chapters will be solicited each year to furnish names of candidates for this award.  This nomination will include a letter of support from the local chapter president, a CV of the nominee and list of accomplishments.  

Awards Committee:   The Chair will compile the entries and send copies to each Award Committee member with an accompanying assessment sheet for evaluation.  The Awards Committee will forward completed assessment sheets to the Chair for review and compilation. A summary will be presented to the NYSCHP BOD for a vote at the next BOD meeting after the application deadline.

Criteria:  Recognizes an individual pharmacist for his or her leadership, vision and continuous contribution to  the practice of pharmacy in New York State in their first 1-5 years after formal education or residency training.  This award is meant to foster involvement from the beginning of their career.

•    Must be a practicing pharmacist in the New York State
•    Must be a pledge level or active member of New York State Council of Health-system Pharmacists.
•    Practicing 1 to 5 years from education or following pharmacist residency training programs (PGY1 or PGY2).
•    Demonstrates support of NYSCHP through active involvement either on a state or local level.
•    Demonstrates superior leadership, innovation, or clinical skills in the practice of Pharmacy.
Presentation:  The award will be presented at the Annual Assembly installation banquet.

This award does not need to be given every year.

The NYSCHP BOD has the ability to add one time discretionary awards as needed to recognize the outstanding contributions of an individual or organization to the NYSCHP or healthcare.
